The unsettling case was shared publicly by HONG Cheng-Hsien, Deputy Chief of Education and Training at Chang Bing Show Chwan Memorial Hospital in Taiwan. The elderly patient initially presented with concerning symptoms including shortness of breath and severe edema in her extremities over a six-week period. Initial lab results indicated highly elevated protein levels in her urine and dangerously low serum albumin—markers suggestive of significant renal distress, despite her glomerular function appearing structurally normal. Though she initially received treatment for a suspected urinary tract infection, her kidney function rapidly deteriorated, necessitating an urgent transfer to a major medical facility.
The Hidden NSAID Danger
Further investigation by physicians revealed she was suffering from a combination of Nephrotic Syndrome and interstitial nephritis. Clinically, the simultaneous occurrence of these two conditions often points toward a link with the use of NSAIDs. However, both the patient and her family repeatedly denied taking any oral pain medication, and external medical records confirmed no relevant prescriptions. The mystery was finally solved when a sharp-eyed nurse discovered three emptied 60g tubes of pain relief gel (piroxicam gel, a potent NSAID) hidden in the patient’s bathroom cabinet. Confronted with the evidence, the patient admitted to applying the cream liberally—twice daily—in the hospital bathroom to manage her chronic muscle aches. Over the six weeks leading up to her renal crisis, she had absorbed an accumulated dose of NSAID far higher than anticipated for topical use.
Dr. Hong warned that while topical creams are often perceived as safe, frequent, expansive application of NSAID gels allows significant drug absorption through the skin into the bloodstream. This chronic exposure can cause nephrotoxicity, particularly in vulnerable populations like the elderly or those with even mildly compromised kidney function. Fortunately, once the source was identified and the topical NSAID discontinued, the woman’s condition dramatically improved. Within three weeks, her edema subsided, proteinuria decreased, and her kidney function nearly returned to normal, eliminating the need for further dialysis.
